Can I order my own lab tests?

Lab testing must be ordered by a licensed healthcare professional or authorized designee licensed in the state of California.

Can I take my medication when the doctor tells me to fast?

Yes. Always take any medications that are part of your routine unless your doctor indicates differently.

How can I find out if my insurance company has paid this claim?

Please review your bill carefully. A line item adjustment will be printed on your bill if we have received payment from the insurance company. If you are still uncertain, you can either contact your insurance company directly or contact the Billing Department at 866-809-9048.

How often will PDL bill me?

PDL will send you a bill approximately once per month. Payment from you is expected promptly upon receipt of the bill. If you have questions about your bill, please contact our Billing Department immediately at 866-809-9048.

I do not have insurance. Do I have to pay up front or can I be billed?

PDL will give you a discount on your laboratory testing if you pay up front through our Time of Service payment program. Please notify the receptionist at the PDL Patient Service Center that you would like to pay at the Time of Service to receive a discount of up to 40% on your lab testing.

What do I do if the information shown on the PDL bill does not match the information I’ve been provided by my insurance company?

If you have received an Explanation of Benefits (EOB) from your insurance company that shows different information than the laboratory bill you have from PDL, please contact the PDL Billing Department at 866-809-9048.

What insurance plans do you accept?

PDL is contracted with most PPO insurance companies and some HMO insurance plans. PDL accepts Medicare and Medi-Cal. To see a full listing of our contracted insurance companies, please click here.

What items do I need to take to bring to my visit to PDL?

When visiting PDL for a specimen collection, please bring the following information with you:

  • The test request form from your healthcare provider
  • Your current insurance identification card
  • Your photo ID card
  • If you are uninsured and paying for your lab work at the Time of Service, please bring a check, a credit card (VISA, Mastercard, Discover) or a money order.

Please note: Minors must be accompanied by their parent or legal guardian for lab testing. If you are bringing a minor and you are not that person’s legal guardian, you will need to bring a notarized statement signed by a parent or legal guardian authorizing you to give permission for the collection and billing of the tests ordered by the minor’s physician.

What methods of payment does PDL accept?

PDL accepts payments by check, money order, or credit card. PDL accepts Mastercard, Visa, and Discover.

Please remember to attach the stub of the bill when mailing in your payment, and include your invoice number on the check.

Whom do I speak with about the results from my lab testing?

The laboratory report is sent to the ordering physician or other authorized person who has requested the test. Please contact your ordering physician for further information regarding your results.

Why didn’t my insurance pay this claim?

Each insurance company processes claims differently. You will receive an Explanation of Benefits (EOB) from your insurance company that details the services and whether the services were paid or denied. If you need additional assistance in determining why your insurance did not pay on your claim, please contact your insurance carrier directly.

Why do I need to provide billing information every time I come in for a visit at PDL?

PDL does not keep files on individual patients; each date of service is treated independently. Therefore, we require complete billing information each time testing is requested.

Why have I received a bill from PDL?

PDL has received information from either your ordering physician or your insurance company indicating there is a patient responsibility portion for testing services. For further information about the specific nature of the bill, please contact our Billing Department at 866-809-9048.
